Do You Qualify?
Here’s how to qualify for our services: ¶ Live in Caldwell, Lyon, Crittenden counties (or Hopkins County with a Dawson Springs address.)Here’s how to qualify for our services: ¶ Live in Caldwell, Lyon, Crittenden counties (or Hopkins County with a Dawson Springs address.) ¶ Are uninsured and are at or below 200% of the Federal Poverty Level. ¶ Are self-insured with a minimum $2,000 per year deductible. ¶ Are employed. ¶ Are attending school but not working. ¶ You must Bring ¶ 1.
